Mental Health Multidisciplinary Team
Role Of Service
The Mental Health MDT was created to provide holistic care to patients with mental health problems who do not meet the threshold for secondary care. The Mental Health MDT can support with diagnostic and medication reviews, onward referrals to services for social, mental, and physical health support, provide patients with tools to help them with their mental health and much more.
Patients must be 18 or over to be referred into this service.
The Mental Health MDT can help you with the following:
- Support shared decision making
- Facilitating onward access to treatment services
- Providing brief psychologically informed intervention
- Treating metal health problems effectively through evidence based
- approaches
- Improving quality of life
- Maximising continuity of care with a more flexible system that proactively
- responds to ongoing care needs
- Improving occupational function through everyday activities
How to be referred to our Mental Health MDT Team:
Book an appointment with your GP to discuss this service and decide if this service would be beneficial to you. If it is decided that it is, your GP will make a referral and this will be reviewed by the team, if your referral is accepted you will be contacted by a member of the team for an initial assessment. This can be done face-to-face or over the phone, depending on which one you prefer.
